CARE COORDINATION FOR CHILDREN AND YOUTH
Our Mental Health Care Coordination service ensures seamless, comprehensive care by connecting you with the right providers, resources, and support systems. We develop personalized care plans, manage referrals, and advocate for your needs, ensuring consistent communication among all parties involved. Our goal is to enhance your mental health by providing integrated, continuous care tailored to your unique situation.

To be eligible, youth must be:

Age 17 and below at time of enrollment, may receive services through age 21.

Diagnosed with or in need of assessment a mental health diagnosis.
TARGETED CASE MANAGEMENT
TARGETED CASE MANAGEMENT(TCM) PLUS
1915i WAIVER
Care Coordination is provided at three levels based on
youth and family needs:

General: up to 3 hours per month

Moderate: up to 7.50 hours per month

Intensive: up to 15 hours per month
TCM Plus aims to provide care coordination and additional
supports to children/youth who have a combination of risk factors
and intensive mental health needs. This includes additional family
peer support and funding for goods and services.
The 1915(i) program provides an array of diagnostic and therapeutic mental health services, including 24-hour availability of mental
health and crisis services.

A licensed mental health professional must complete a comprehensive assessment outlining how the young person presents a potential
danger to themselves or family, friends and community members.
